Youth Ministry Forms & Mailings

The following are forms for a variety of programs/events/activities. Each of these forms is in PDF format (unless otherwise noted):

2008-2009 Religious Education Registration Form (for K-11 RE, not including Confirmation) – This form is to be completed by all who attend the Religious Education program in grades K through 8 (high school students can also be put on this form, especially if she/he has younger siblings in the program).

2008-2009 High School Handbook & Registration Form – This handbook contains a listing of all the segments that are offered at Blessed Sacrament for those in 9^th & 10^th Grade (pre-Confirmation). Also, a registration form on the 4^th page must be completed by each student in the program, indicating which segments he/she are planning to attend. 2008-2009 Handbook & Registration Form coming soon.

Salvation Army Volunteer Sheet & Permission Form - This provides information and the parent permission form for the Salvation Army volunteering opportunity during the 2007-2008 school year.
